Physician care
Our physicians ensure that all treatments align with the patient’s values and wishes, offering a holistic approach to end-of-life care. With a focus on comfort and quality of life, they provide the medical expertise necessary to navigate this complex journey.
Nursing care
Our nurses focus on the comfort of every patient with an individual approach to everyone. Registered nurses specialize in pain management and symptom control, ensuring patients remain comfortable and dignified throughout their care.
Aide care
A hospice aide provides compassionate, hands-on support, assisting with personal care, comfort, and daily living activities under the supervision of a nurse, while helping patients maintain dignity and quality of life.
Spiritual care
Our chaplains and spiritual counselors offer support that transcends all faiths and beliefs, providing a comforting presence during this sacred time. They are available to listen, offer prayers, or simply be a reassuring presence.
Social care
Our medical social workers provide emotional, social, and practical support: coordinating care, connecting families with community resources, assisting with advance care planning, and providing counseling.
Volunteer care
Our trained volunteers provide companionship, comfort, and support: friendly visits, emotional support, respite for caregivers, and help with non-medical tasks.

How it works
One plan of care, reviewed together.
Within the first days of care, the team meets with the patient and family to build an individualized plan of care. The interdisciplinary team then meets at least every two weeks to review symptoms, goals, and what’s working, and updates the plan as needs change. Between visits, a real hospice nurse is a phone call away, 24 hours a day.
Admission visit
A registered nurse evaluates comfort needs, explains the benefit, and starts medications and equipment, often the same day.
Plan of care
The team, the patient, the family, and the attending physician agree on goals, visit frequency, and what to expect.
Regular visits
Nurses, aides, social workers, chaplains, and volunteers visit on a schedule built around the patient, adjusted as needs change.
