We come to the home, assisted living, memory care, or skilled nursing facility, and coordinate closely with facility staff.
Support around the clock
Sundowning and nighttime distress don’t keep office hours. A hospice nurse is one call away, any hour.
Dignity and personhood
We honor each person’s history, culture, language, and preferences, and help families connect in the ways that still bring joy.
Grief support along the way
Families often grieve long before a loss. Our social workers and chaplains support that journey and continue for 13 months after.
When to call
Signs that hospice or palliative support may help.
Families living with advanced dementia often wait too long to ask for help. Consider calling us when you notice:
- Difficulty walking, sitting up, or holding the head up without help
- Trouble swallowing, weight loss, or reduced interest in eating
- Recurrent infections, pneumonia, or hospitalizations
- Speech limited to a few words, or little response to surroundings
- Caregiver exhaustion, physically or emotionally
