The four levels of hospice care.

Hospice care isn’t one-size-fits-all. As a Medicare-certified hospice, Caring and Wellness Hospice Care provides all four levels of care defined by Medicare, and we move with your loved one between them as needs change, so the right level of support is always in place.

Most common

Routine Home Care

Wherever you call home

This is how we deliver hospice care most often: right where the patient lives, whether that’s a private home, an assisted living community, or a nursing facility. Our team makes regular visits to manage symptoms and provide nursing care, personal care from aides, medications, medical equipment, and emotional and spiritual support.

How we provide it: Scheduled visits from your care team, with a real hospice nurse available by phone 24 hours a day, every day of the year.

Crisis care

Continuous Home Care

In the home, during a crisis

When a medical crisis strikes (a surge in pain, trouble breathing, severe nausea, or agitation), we provide intensive, mostly nursing care at the bedside in shifts of up to 24 hours a day. The goal is to bring the crisis under control at home and avoid an unnecessary hospitalization, then ease back to routine care once symptoms are stable.

How we provide it: Our nurses stay at the bedside through the crisis, adjusting medications and treatments until your loved one is comfortable and steady again.

Acute symptom management

General Inpatient Care

At a partner facility

If symptoms can’t be safely managed at home, we arrange short-term inpatient care at a Medicare-approved partner hospital, skilled nursing facility, or hospice inpatient unit. There, the patient receives around-the-clock attention to bring severe pain or symptoms under control, with the goal of returning home as soon as they’re stable.

How we provide it: We coordinate the transfer and continue directing the plan of care, then guide the move back home when the time is right.

Support for caregivers

Respite Care

Up to 5 days, at a facility

Caring for a loved one is demanding, and rest matters. Respite care provides a short break of up to five consecutive days by caring for the patient in a Medicare-approved facility so the primary caregiver can step back and recharge, knowing their loved one is in attentive, capable hands.

How we provide it: We arrange the respite stay and handle the logistics, so taking a needed breath is simple for your family.

Care that moves with you

One level, or all four, whatever the moment calls for.

Most patients spend the majority of their time in routine home care, with the other levels available the moment they’re needed. Our team continually assesses your loved one’s needs and, together with the hospice physician, recommends the right level of care at the right time, so comfort and dignity are always protected.
